Enough that someone who was absent understands what was decided and why, without transcribing the conversation. Record the data reviewed, the decision, the rationale, and the action. Leave out the back-and-forth. Minutes that read as a transcript are rarely read at all, which defeats their purpose as a record.

Be careful here. For an incident review, the committee is looking at causes and systems, and naming a driver in a widely circulated document can create employment and privacy issues. Refer to the incident reference and keep personal detail in the investigation file, which has controlled access. Follow your own HR policy and, where the stakes are high, take advice.

Because re-dating an overdue action makes it look current and hides the slippage. Keep the original date, add the revised one, and record the reason. Three revisions on one action is a signal the committee should act on — either the owner lacks capacity or the action was never realistic.

Keep them for as long as your safety records generally, and longer where they evidence how a known risk was handled. Minutes are frequently requested after a serious incident or in litigation, precisely because they show what the organisation knew and when. Retention periods vary by jurisdiction, so set yours deliberately rather than by default.
