Record the current intervention threshold next to each category rather than from memory; thresholds differ by category and by vehicle type (passenger vs. property carriers).

Quick answers to the questions buyers usually ask once the category, software, or rollout details start getting more specific.
It means your carrier's rate of relevant violations, weighted by severity and recency, has worsened relative to other carriers with similar exposure — not a raw violation count. A percentile can rise even with the same number of violations if peer carriers are improving faster than you are, which is why tracking the trend and its contributing causes matters more than reacting to any single percentile figure.
No — thresholds differ by category, and some also differ between passenger and property carriers. Confirm the current threshold for each category directly rather than assuming they're uniform; treating every category as having the same cutoff is a common and easily avoided mistake.
As far as the scoring window itself looks back, which weights recent violations more heavily than older ones but still includes a multi-year history — a percentile change can be driven by an older violation finally aging out of the window as much as by a new one. Check your full inspection history for the category, not just the most recent month.
A percentile above threshold makes a carrier eligible for FMCSA intervention, which can range from a warning letter to a more involved investigation, but it isn't an automatic trigger for every carrier every time. Treat crossing a threshold as a serious signal to act on the underlying cause regardless of whether an intervention letter has arrived yet.
