It's a graduated approach — typically a documented verbal warning, then a written warning, then a final warning, then termination — that gives the employee notice and a chance to correct. This form supports each step. Align the steps to your own handbook policy.
A signature usually acknowledges receipt, not agreement. If the driver declines, note that on the form, have a witness initial it, and still provide a copy. Consult your HR policy and counsel for sensitive cases.
A DOT-reportable accident, drug/alcohol violation, or roadside out-of-service event has its own compliance records. This warning form documents the HR/performance side and can reference those records, but it doesn't replace them.
Retention depends on your jurisdiction and policy. Many employers keep them for the duration of employment plus a defined period. Confirm with HR and counsel what applies to you.
