What you get

  • Driver and date header fields
  • An itemized list of the policies the driver is acknowledging (safety, vehicle use, drug & alcohol, phone use, HOS)
  • An at-will / not-a-contract statement placeholder
  • A statement that the driver agrees to follow the policies and ask questions if unclear
  • Driver and witness signature blocks
  • An editable Word file so the policy list matches your actual handbook

How to use it

  1. 1

Customize the policy list so it matches the sections in your current driver handbook.

  2. 2

Give the driver the handbook and time to read it during onboarding.

  3. 3

Walk through the high-risk policies (safety, drug & alcohol, device use) and answer questions.

  4. 4

Have the driver sign and date the acknowledgment confirming receipt and agreement.

  5. 5

File the signed form in the personnel record and re-issue it whenever the handbook is materially updated.

Frequently asked questions

Quick answers to the questions buyers usually ask once the category, software, or rollout details start getting more specific.

A

If a driver violates a policy, the signed acknowledgment is your evidence that they received and understood it. It supports consistent enforcement and progressive discipline. It does not, by itself, create or remove employment rights — confirm the wording with counsel.

A

Re-issue the acknowledgment whenever you materially update the handbook or its key policies, and many fleets re-confirm annually. Keep each signed version with its revision date so you know which policy the driver agreed to.

A

Some policies (like the drug & alcohol policy) benefit from their own dedicated sign-off. This form acknowledges the handbook as a whole and lists the key policies, but you can keep separate acknowledgments where the stakes are high.

A

Most handbooks include language clarifying the handbook is not an employment contract and that employment is at-will where applicable. Jurisdiction rules vary, so confirm the statement with counsel before using it.
