How to use it

    Fill the header with the date, driver, carrier, and unit numbers before the shift.

    Draw a continuous line across the grid, stepping between the four duty statuses as they change.

    Note the city/state at each status change in the remarks line.

  4. 4

    Total the hours in each status; the four totals must sum to 24.

  5. 5

    Update the daily recap with your running cycle hours and sign to certify the record.

Most drivers required to keep records of duty status must use an ELD, but exceptions exist (e.g., certain short-haul operations, pre-2000 engines, limited driving-day counts). Paper logs are also allowed as a temporary backup when an ELD malfunctions. Verify your status against 49 CFR 395.
